Interviewing my collaborators at work

I am in this part of year when one must evaluate and, as a result of that, interview every person of my team. Every time this team gets even greater, so I needed to find out a way to reduce time (I am pretty much focused on high performance in every task in my job).
Creatting a pattern, so that every interview is consistent with the other ones could be a way to simplify, nevertheless every interview is unique, because every person and her work are unique as well.
Hereby it is the model I wrote on the plane in order to get these interviews´ framework.
